Every beat of your heart is powered by energy made deep inside your cells — and one nutrient sits right at the centre of that process. It's called coenzyme Q10, and your heart needs more of it than almost any other organ. Here's what it actually does, how CoQ10 supports heart health, why age and statins drain it, and where the evidence for the CoQ10 heart benefits is strongest.
What is CoQ10?
Coenzyme Q10 (CoQ10), also known as ubiquinone, is a vitamin-like compound your body makes itself. It lives in the mitochondria — the tiny "power plants" inside every cell — where it plays two essential roles: it's a crucial link in the chain that turns food and oxygen into cellular energy (ATP), and it's a potent fat-soluble antioxidant that helps protect cell membranes from oxidative damage. In short, CoQ10 helps your cells both make energy and defend themselves while doing it.
A useful detail: CoQ10 cycles between two forms as it works — ubiquinone (oxidised) and ubiquinol (reduced) — shuttling electrons in the energy chain and, in its reduced form, quenching free radicals. It's one of the few antioxidants the body both makes and can regenerate, and it sits in every cell, which is why a shortfall is felt body-wide rather than in one place.
Why the heart depends on it
Because CoQ10 fuels energy production, the organs that need it most are the ones that work hardest — and nothing works harder, or more relentlessly, than the heart. Heart muscle is packed with mitochondria and carries some of the highest CoQ10 concentrations in the body. When CoQ10 runs low, the heart's energy supply and antioxidant defences both take a hit, which is why coenzyme Q10 heart health has drawn far more research than any of its other uses.
A 2024 meta-analysis of 33 randomised controlled trials found that, added to standard treatment, CoQ10 significantly reduced all-cause mortality and hospitalisations in people with heart failure, and improved measures of heart function (Xu et al., 2024). It's studied as a supportive add-on, not a replacement for prescribed heart treatment.
Why your levels fall — age and statins
Two things commonly lower CoQ10. The first is simply age: the body's own production peaks in our twenties and declines steadily thereafter, so older adults tend to have less. The second is more specific — statins, the widely prescribed cholesterol-lowering drugs. Statins work by blocking an enzyme pathway that the body also uses to make CoQ10, so they can reduce CoQ10 levels as a side effect. This has led to interest in whether topping CoQ10 back up can ease the muscle aches (statin-associated muscle symptoms) that lead some people to stop taking their statins.
A 2024 systematic review of randomised trials on CoQ10 for statin-induced muscle symptoms found the results mixed — several trials reported reduced muscle pain with CoQ10 while others found no significant change (Ahmad et al., 2024). Given CoQ10's strong safety record, it's often tried as a low-risk option for statin-related muscle symptoms, ideally alongside — not instead of — a conversation with your doctor.
CoQ10 levels aren't routinely tested, so most people supplement based on age, medication or symptoms rather than a blood result — another reason to treat it as a considered choice rather than a default.
Beyond the heart
CoQ10's role in energy and antioxidant defence means it's been studied more broadly — for blood pressure, exercise performance and recovery, migraine prevention, fertility (egg and sperm quality), and general "cellular energy" in fatigue. The evidence across these is uneven and often early, so they're best seen as areas of genuine interest rather than settled benefits. The clearest, most consistent signal remains the CoQ10 cardiovascular benefits, particularly in heart failure and in people who are depleted.
The migraine research is perhaps the most notable of the secondary uses: several studies suggest regular CoQ10 can modestly reduce how often migraines strike, which is why it features in some preventive protocols. And because mitochondria power everything from muscle to brain, the interest in CoQ10 for fatigue and recovery is mechanistically reasonable even where large trials are still lacking.
Who might consider CoQ10
CoQ10 isn't something everyone needs to supplement, but a few groups have a clearer rationale: older adults, whose natural production has declined; people with heart failure or cardiovascular disease, who may take CoQ10 for cardiovascular support as a doctor-supervised add-on to standard care; statin users troubled by muscle aches, given statins' effect on CoQ10; and those exploring migraine prevention or exercise recovery, where the evidence is lighter but the safety is reassuring. For a healthy younger adult eating well, the case is weaker — CoQ10 is most useful where age, medication or a heart condition has tipped the balance toward depletion.
Ubiquinone or ubiquinol — and how to take it
You'll see CoQ10 sold in two forms: ubiquinone (the oxidised, more stable form used in most research) and ubiquinol (the reduced, "ready to use" form some prefer, particularly older adults). Both raise CoQ10 levels; the practical difference is smaller than marketing suggests. The bigger issue is absorption — CoQ10 is fat-soluble and notoriously hard to absorb, so it should always be taken with a meal containing some fat, and well-formulated (for example, liposomal) preparations are designed to improve uptake.

Always with food containing some fat, since it's fat-soluble and poorly absorbed on its own. A well-designed formulation (such as a liposomal one) helps it reach the bloodstream. Consistency day to day matters more than large occasional doses.
Is CoQ10 safe?
CoQ10 has an excellent safety profile and is generally very well tolerated. If you take blood-thinning medication (such as warfarin) or heart medicines, check with your doctor first, as CoQ10 can occasionally interact.
